The purpose of this CDC project is to better meet the needs of agencies serving high-risk and/or racial/ethnic minority populations requesting Evidence-Based Interventions,-Public Health Strategies capacity building assistance (CBA), this program proposes to implement a four step plan that includes 1) a comprehensive assessment to accurately identify the problem or issue, identify agency strengths and CBA needs; 2) an action plan that utilizes logic modeling and lays out steps to address CBA needs; 3) culturally-appropriate CBA designed to address identified problem(s) and related factors; and 4) monitoring and evaluation (M&E) based on the logic model and process and outcome measures. PHFE will act as the applicant for this proposed program and will act as the fiscal agent providing the customary back office services.
